Honors Thesis Student/Sponsor Agreement (pdf)
This form is to be completed during pre-registration of the semester prior to registering for the thesis. This form serves as notice to the UHP that the student will be eligible for a University Honors designation.

In order for us to keep track of your progress in completing program requirements, we ask seniors to submit this form to our office in early fall.
You may want to use this form to monitor your own progress in the program.
Certification of Completion of Honors Program Requirements and Senior Questionnaire (pdf)
In order to graduate with an honors designation from the Honors Program, the Certification of Completion of Honors Program Requirements form and Senior Questionnaire must be completed and submitted to the Honors Program Office.
Permission to Examine Thesis (pdf)
This form is completed when you turn in a copy of your thesis (on your chosen media) to the UHP office. It allows us to store your thesis at Dimond Library, Special Collections & Archives, and provides other students access to your thesis. Your signature must be witnessed by a member of the UHP office.
Honors Designation Form (pdf)
In order to designate a course "honors," you need to complete the Honors Designation Form and submit it to the Registrars Office by

Honors Credit for Study Abroad
In order to receive "honors" credit for Study Abroad/Category 5 (the Foreign Culture General Education requirement), you must complete INCO 650H in the semester prior to leaving for your study abroad experience and complete INCO 651H in the semester following your return. Both courses are 1 credit, Pass/Fail and are seven weeks in duration.
After successfully completing INCO 651H you'll need to fill in and submit the Study Abroad Category 5 UHP form to the UHP office.
